The information obtained will be used to determine whether or not your qualifications meet those necessary to be included on the bid list. A detailed inquiry may be required prior to award of a project.

- Firm’s full name, address, and contact information
- Attach MBE or WBE certificate(s) if applicable
- List firm’s Principles, project managers, estimators, and staff
- Year and state of incorporation. Current and name and other name that may be applicable.
- Type of work – Ex: 09250-Drywall, 15600-Mechanical
- Check appropriate self-performance box
- Engineering Capabilities? If yes, please provide state where licensed, type, and general liability
- All applicable geographical areas where your firm performs work
- Average sales for the 3 years
- Project size (Min-Max)
- Bonding information
- Written safety program
- References including Projects, Banking, WIP Schedule, & Suppliers
